Advance care planning involves making future healthcare decisions that include much more than deciding what care you would or would not want; it starts with expressing preferences, clarifying values, identifying health care preferences and selecting an agent to express healthcare decisions if you are unable to speak for yourself.

National Healthcare Decisions Day is a collaborative effort of national, state and community organizations committed to ensuring that all adults with decision-making capacity in the United States have the information and resources to communicate and document their future healthcare decisions.

Advance care planning starts with talking with your loved ones, your healthcare providers, and even your friends- all are important steps to making your wishes known. These conversations will relieve loved ones and healthcare providers of the need to guess what you would want if you are ever facing a healthcare or medical crisis.

National Healthcare Decisions Day (NHDD) is a national initiative to encourage adults of all ages to plan ahead of a health crisis. Making decisions ahead of time and putting your wishes in writing brings peace of mind to families. It helps to avoid the difficult situations that are so common when a person becomes seriously ill and the family is left to guess what their loved one would have wanted. Another goal of this annual event is to encourage health care providers to discuss the topic with their patients.

Five Wishes is a comprehensive, person-centered advance care planning program that offers healthcare providers a proven, easy-to-use approach to having effective and compassionate conversations.

National Healthcare Decisions Day is a day of awareness and education. It’s focused on encouraging everyone – including those who are healthy and in the prime of life – to think about and document their healthcare treatment wishes before a crisis.

Why is this so important? Completing your Advance Directives – which in Ohio includes Health Care Power of Attorney and Living Will forms – empowers you to have a voice in your care by communicating your preferences based on your wishes, values and goals.

Making these choices while you are still healthy and then sharing the documented plan with loved ones and doctors is the best way to make sure your choices will be honored should you become unable to speak for yourself.

Planning for future healthcare choices does not happen overnight, but it need not be a scary process. It can start with a family conversation around the dinner table. Allow time for thought and consideration of the options. Sharing your choices through conversation is an important first step. In the long run the conversations will be the greatest gift you can give to those you love. It will give them the confidence to act knowingly on your behalf and provide the comfort of knowing that your wishes will be honored.
